This course consists of four modules and are designed to give you information on how to implement a program within your own agency to successfully transition individuals with behavioral health conditions and medical challenges out of institutional living into community living. The information contained in these modules is informed by the Money Follows the Person Behavioral Health Pilot.
Upon successful completion of the course, you will earn a certificate of completion, but no continuing education credit is offered.
Module 1 is designed to provide you with of an overview of the Money Follows the Person program and it's benefits.
Module 2 is designed to describe the responsibilities of a Cognitive Adaptation Training or CAT Therapist and the factors influencing the type and the amount of time spent on CAT interventions before and after Nursing facilities transitions.
Module 3 is designed to provide you with an understanding of the importance of collaboration to support individuals with serious mental illness who transition from a nursing facility into the community.
Module 4 is designed to provide you with an understanding of the relocation process from the perspective of the CAT Therapist when supporting individuals with serious mental illness (SMI) who are transitioning from a nursing facility into the community.
