This virtual training is being sponsored by the HHSC Residential Treatment Center (RTC) Project. Staff of children’s RTCs contracted with the HHSC RTC Project have priority for this training. If you do not work for or with a Children’s RTC contracted with HHSC via the RTC Project, you are not eligible to register/attend.
This virtual training provides child welfare and residential care professionals with practical, trauma-informed strategies to support effective transition and discharge planning. Participants will learn essential components of high-quality plans, explore collaboration techniques, and practice applying tools that promote stability, permanency, and successful re-entry into family and community settings.
Learning Objectives:
Participants will learn to:
1. Explain the purpose and requirements of transition & discharge planning in child welfare residential settings.
2. Identify essential components of an effective transition plan.
3. Use trauma-informed, youth-centered strategies to prepare young people for step-down or permanency.
4. Collaborate effectively with case managers, caregivers, families, and community supports.
5. Apply best practices using real-world scenarios.
6. Create individualized, measurable, and stable transition goals.

• A colleague may be substituted at no extra charge if Centralized Training is notified at least two business days before the training.
• Requests for refunds for an in-person/virtual workshop must be received 72 hours prior to the workshop.
Self-Paced Online Trainings:
• No refunds are offered for self-paced online trainings.
